Short description
The Child and Young Person Neurodevelopmental Coordination Service aims to provide children, young people and their families living in North East Essex with access to early help and navigational support as they enter, pass through or complete a diagnostic assessment pathway.
The Service provides simple signposting to a partner agency, a formal referral onwards to a partner agency, discussion and joint planning with a Health Visitor or School nurse or instigating a team around the family meeting organised by the case load holder so that multiple partners can create a robust plan of support for the client. The coordinator will follow up each client´s pathway individually at intervals dependant on the provision advised varying between 4 to 12 weeks in duration based on complexity of the plan provided.
